State Licensure Guide
State licensing is an important step in your career search. Some practices (especially in the state of Texas and other states with long timelines for licensing) will not take your application seriously if you don’t have or are not in the process of applying for a license. We have done the research for you to try and help you understand the requirements, cost and timeline for each state. Cost, timeline and requirements vary greatly between states.
Timeline: On average, state licensing takes 8-12 weeks. However, some states can take up to 9 months!
- Cost: On average, state license applications cost about $500. Some of the more expensive states can cost over $1,000
Length of Time to Receive License
Six to eight weeks, on average. Once an application for certificate of qualification (COQ) is received, it will be added to the agenda of the next upcoming Board meeting for review. This timeline may be increased should the Board need to request further information about the application.
Application Highlights
Once the application for certificate of qualification (COQ) has been submitted, the applicant will be mailed the Medical Licensure Commissiion’s application, which must be completed in order to receive an Alabama Controlled Substance Certificate. This must be returned, along with a $75 fee, within six months or the application with expire. This information must be received before a license number will be issued.
Some sections of the application may accept the FCVS.

Length of Time to Receive License
12-14 weeks, on average. This time may be extended depending on how long it takes to receive all required documentation. Applications are reviewed at the next scheduled Board meeting. Board meetings occur quarterly.

FCVS & the UA are both accepted by the State of Alaska. All application documentation is valid for six months before it expires.
In general, the Board reviews applications in the order in which they were received. However, should there be a situation that would “result in a termination of service to patients” a facility executive may request expedited review.

Licenses are valid for two years and expire on 2/28 of even numbered years.
Length of Time to Receive License
2-4 months, on average. Applications are reviewed at quarterly Board meetings.
Application Highlights
Wyoming uses the Uniform Application (UA). Applications must be completed online. State-specific affidavit and addendums must be submitted via mail.
FCVS is required.
A temporary license may be issued once all application pieces have been submitted and if the phsycian meets certain requirements. There is no additional cost for a temporary license.
Applications expire after 6 months.
